Support Coordination

Making your NDIS plan work for you

We help participants understand and implement their NDIS plans, connecting them with the right supports and services to achieve their goals.

Overview

What is Support Coordination?

Who we support

  • Participants with complex support needs across multiple providers
  • People new to the NDIS who need help getting started
  • Participants whose circumstances have changed significantly
  • People transitioning from hospital, residential care, or justice settings

Support Coordination is a capacity-building support that helps you get the most out of your NDIS plan. Our experienced coordinators work alongside you to understand your goals, identify the right providers, and ensure your supports are working together effectively. We take the complexity out of navigating the NDIS so you can focus on living your life.

Frequently asked questions

Still have questions?

Our team is happy to answer any questions about Support Coordination and how it can support you or your participant.

  • Support Coordination helps you implement your plan and connect with providers. Specialist Support Coordination is for participants with more complex needs — it involves a higher level of expertise and is typically funded separately in your plan.

  • Support Coordination is funded under the Capacity Building — Support Coordination budget in your NDIS plan. Not all plans include this funding — if you believe you need it, speak with your LAC or NDIA planner at your next review.

  • Yes. You can change your Support Coordinator at any time. We recommend giving reasonable notice and ensuring a proper handover so your supports are not disrupted.

  • This depends on your needs and plan funding. Most participants meet with their coordinator monthly, with additional contact during plan reviews, transitions, or when issues arise.
